All guests are required to submit this form prior to receiving any service at Elle Marie Hair Studio.








I understand that I am choosing to receive an elective service that is not urgent and is not deemed essential.

The coronavirus, COVID-19, has been classified as a worldwide pandemic by the World Health Organization. I further understand that COVID-19 is extremely contagious and is believed to spread by person to person contact; and, as a result, federal and state health agencies recommend social distancing. I recognize that my stylist and the entire staff at Elle Marie Hair Studio are closely monitoring this situation and put in place reasonable preventative measures aimed to reduce the spread of COVID-19.

Given the nature of the virus, I understand there is a risk of becoming infected with COVID-19 if I attend the salon for an elective service. I hereby acknowledge and assume the risk in becoming infected with COVID-19 through any elective service. Elle Marie Hair Studio is not held responsible for any potential impact on my health. I understand that possible exposure to COVID-19 before/during/after my service may result in the following: positive COVID-19 diagnosis, extended quarantine/self-isolation, additional tests, hospitalization that may require medical services, intensive care treatment, possible need for intubation/ventilator support, short term or long term intubation, other potential complications and the risk of death. I have been given the option to defer my services to a later date. However, I understand all potential risks, including but not limited to the potential short-term and long-term complications related to COVID-19, and I would like to proceed with my scheduled service at Elle Marie Hair Studio.


I agree to wear a mask or other facial covering for the entire duration of my service. I understand that if I do not provide my own mask, I will be asked to purchase one for $2 + tax before my service can begin.

I agree to have my temperature checked by a Guest Service Administrator prior to entering the establishment. A temperature of 100.4 F or higher is considered a fever. Anyone who has symptoms of influenza, COVID-19, or the common cold, or whose temperature exceeds 100.3 degrees will be required to reschedule their service for another time without penalty.

I agree to return to my vehicle after checking in to wait for my stylist. A team member will call or text me to enter the salon when my service can begin.

I agree to disinfect my hands immediately upon entering the salon.

I agree that we will not be sharing hugs or handshakes at this time. Virtual and air hugs are encouraged!

I agree to only bring items into the salon that are essential for my service, such as payment, phone, and a beverage. At this time, all other personal items must be left at home or in a vehicle, including coats, purses, food, laptops, etc.

I understand that my salon visit may take up to an additional 15-30 minutes due to sanitation guidelines and extra services needed.

I agree to arrive alone for my appointment. I understand that only guests actively receiving a service will be allowed inside the salon. Any violation will result in a refusal of service. For guests under 18, a parent or guardian may be present for the consultation only. Appointments for children who need a parent present will be determined on a case by case basis.

I agree to follow proper social distancing measures and maintain a distance from others of at least 6 feet whenever possible.

I understand that Elle Marie will only provide capes as protective wear at this time. I understand that additional protective wear, such as the color robes typically worn during chemical services, will not be provided at this time. I have been informed that it is suggested to wear something that may get color on it. Elle Marie Hair studio assumes no responsibility for clothing items that are damaged in any way during my service.

During the Stay Home, Stay Healthy order, I may have provided myself or had someone else provide me with hair services that may interfere with my wanted end results in the salon. I understand that a redo may not be applicable during this time and that reaching my goals may take time and not necessarily have a favorable outcome in between hair appointments.

I agree to pay for my service with a credit or debit card if at all possible. I understand that cash payments are STRONGLY discouraged at this time.

I agree to comply with any additional safety measures at the time of service as directed by Elle Marie Hair Studio.


If I do not show up for my reserved appointment, I agree to pay 50% of the cost for all services scheduled upon the first occurrence. I understand that this No Show fee is nonrefundable except when documented in an emergency situation. I also acknowledge that upon my second No Show, my No Show Fee increases to 75% of the service, and upon my third No Show, the fee increases to 100% of the scheduled service.

I understand that Elle Marie Hair Studio requires a 24 hour notice for reservation changes and/or cancellations. Reservations changed or cancelled within 24 hours of their start time are considered a No Show, and I agree to pay for all associated fees.

I understand that if I arrive more than 15 minutes late for my scheduled appointment time, I may not be able to receive my full service. If being late to my reserved appointment time means the service reserved cannot be performed I agree to pay for all associated fees.

I understand that if I No Show for my reservation, I may be asked to provide a 50% deposit for any future reservations at Elle Marie Hair Studio.

I understand that failure to comply with any of the above safety and sanitation requirements is considered a refusal of service and a No Show fee will be assessed.
